Job Description Job Description We are seeking a PRN/Per Diem Physical Therapist to join our Home Healthcare agency team! You will plan physical rehabilitative programs to improve patient quality of life. The ideal candidate will perform comprehensive evaluations, recommend an individualized plan of care, and deliver evidence-based interventions to improve mobility, strength, safety and functional independence, physical therapy service to patients of all ages, with a focus on initial evaluations and re-evaluations. Responsibilities: Conduct comprehensive patient assessments Develop and implement individualized treatment plans Provide hands-on therapy and therapeutic exercises Collaborate with other healthcare professionals PTA referral for follow up care OT referral for assessment and follow up treatment Requirements: Master’s degree in physical therapy Current state licensure Excellent interpersonal and communication skills Ability to work independently and as part of a team Flexible work hours where you create your own schedule Join our team of home healthcare professionals dedicated to improving the lives of our patients through quality care and compassion.
